Every ACORD Form Used in Commercial Insurance

ACORD forms are the standardized application forms used throughout the commercial insurance industry. Most agents use the same 3–5 forms repeatedly, but knowing the full library helps when encountering specialty accounts or coverage lines outside the standard BOP and GL/WC package. Here is a complete reference of the ACORD forms used in commercial insurance and when each is required.

Core commercial application forms

ACORD 125
Commercial Insurance Application
Required on every commercial submission. Covers named insured information, business contact details, prior carrier history, and loss history. The coversheet for every commercial package.
ACORD 126
Commercial General Liability Section
Required for any GL coverage, including BOP GL, standalone CGL, and GL within a commercial package. Captures class codes, revenue, operations description, and premises information.
ACORD 130
Workers Compensation Section
Required for all workers compensation submissions. Captures payroll by class code, officer election/exclusion information, EMR, and prior WC carrier history.
ACORD 140
Property Section
Required for commercial property coverage — standalone or within a package. Captures building location, construction type, year built, protection class, replacement cost value, and coverage options.
ACORD 137
Commercial Auto Section
Required for business auto coverage. Covers the vehicle schedule, driver list, MVR information, and prior auto carrier history. Used with the ACORD 125.

Specialty and supplemental forms

ACORD 131
Professional Liability Section
Used for E&O, D&O, and professional liability submissions where a carrier accepts ACORD format. Many professional liability carriers use proprietary supplementals instead of or in addition to ACORD 131.
ACORD 150
Garage Section
Required for auto dealers, service stations, repair shops, and other garage operations. Covers dealer inventory, service operations, and the unique liability exposures of auto-related businesses.
ACORD 160
Farm Section
Covers agricultural operations — crop, livestock, and farm property. Used by rural commercial insurance agents writing farm accounts.
ACORD 180
Umbrella/Excess Section
Used for commercial umbrella and excess liability submissions. Documents the underlying coverage structure that the umbrella sits above.
ACORD 855
Inland Marine Section
Used for floater policies covering movable property — contractors equipment, installation floaters, motor truck cargo, and other inland marine classes.
